The following items are required to be turned in by one week prior to test date to avoid a late fee:
Online registration of Application For Black Belt Advancement, signed Black Belt test paper and agreement (front and back) signed day of test, electric copy of your essay emailed to frontdesk@manna.us, testing fee paid and four (4) Passport Photos (photos may be scanned and submited in high quality .jpg or .pdf format).
